CLC TN Innovation Coordination Services (ICS)
CLC ICS is committed to offering person-centered approaches to coordinate services with the goal of increasing access to direct services and community based- supports, while supporting those we serve achieve a greater level of independence at home, where they work, and within the communities they live. CLC ICS is a comprehensive, community-based model, that provides wraparound support to individuals and their circle of support, service providers, state partners, and others by implementing the following:
- Innovative Solutions
- Comprehensive Assessments
- Person Centered Planning
- Intentional Engagement and Support
- Strategic Technical Assistance
The following program is offered statewide in Tennessee for all participants enrolled in the Medicaid Alternative Pathways to Services (MAPs) program.
