Case Management

The knowledgeable and experienced Case managers at the Charles Lea Center help people who are eligible for disability resources through the South Carolina Department of Disabilities and Special Needs (SCDDSN), receive the support and care they need. Our goal at the Charles Lea Center is to be a disability resource expert to help people access the services and resources that they need to live as safe, healthy and independent as possible. The Case Manager will assess each person’s needs to help develop an individualized support plan for that person. Case managers also monitor satisfaction and effectiveness of all support services and serve as an advocate for each person to ensure that their rights are protected. The case manager facilitates disability resources in South Carolina to support employment, medical, educational, and social needs. Charles Lea Center case managers receive ongoing education about the numerous disability services and technology available. They attend regular training courses and are well versed per ever-changing state and national guidelines and regulations. The best reward for Charles Lea Center case managers is working with individuals to help them reach their greatest potential. Our Charles Lea case managers strive to provide individualized support to each person based on their wants, needs and goals.

How to Get Services

To receive any of the services offered by the Charles Lea Center, you must have an open case with the South Carolina Department of Disabilities and Special Needs (SCDDSN).
