Customized living

LeaCares is a customized disability care service provided in a person’s home, supporting adults and children with disabilities across Spartanburg, SC, funded through the Department of Health & Human Services (DHHS).

Program Description:

LeaCares is designed to prevent individuals from needing more costly, institutional care in a nursing home or similar facility. Our program serves children and adults with intellectual and physical disabilities through flexible adult care services that enable them to remain at home while addressing all health and personal needs.LeaCares provides highly flexible services and supports that ensures that the individual is able to remain in their home while at the same time ensuring that all the health and personal needs are being met.  These services can be short-term or long-term depending on the needs of the individual.  All plans are highly individualized, flexible and respond to the changing needs of the person.

LeaCares supports several waiver programs through the S.C. Department of Health and Human Services Division of Community Long-Term Care and the S.C. Department of Disabilities and Special Needs, providing community support services and customized in-home care for adults with disabilities.”
