Customized living
LeaCares is a customized disability care service provided in a person’s home, supporting adults and children with disabilities across Spartanburg, SC, funded through the Department of Health & Human Services (DHHS).
Program Description:
LeaCares is designed to prevent individuals from needing more costly, institutional care in a nursing home or similar facility. Our program serves children and adults with intellectual and physical disabilities through flexible adult care services that enable them to remain at home while addressing all health and personal needs.LeaCares provides highly flexible services and supports that ensures that the individual is able to remain in their home while at the same time ensuring that all the health and personal needs are being met. These services can be short-term or long-term depending on the needs of the individual. All plans are highly individualized, flexible and respond to the changing needs of the person.
Program Goals:
- To provide customized in-home disability services to promote independence in daily activities.
- To increase physical, emotional, and intellectual well-being through individualized adult care support services.
- To maintain or increase independence in activities of daily living through personalized disability support services.
- To provide emotional support and assistance as part of comprehensive adult disability care.
- To maintain a daily routine that is personal and desired by the individual, supporting independent living in Spartanburg, SC
- To provide respectful physical care and support through accessible community programs and adult care services.
- To assist with necessary medical and physical care as part of our community support services for adults with disabilities.
Admission Criteria:
- Guardian or individual must agree to receive customized living services from LeaCares
- Services must be in alignment with individual’s goals and preferences
- LeaCares must have the resources and expertise to effectively support the individual
- The individual must be a resident of Spartanburg County, Greenville County, Union County, Cherokee County, or Laurens County in South Carolina to qualify for services.
- The individual will benefit from In-Home disability services
Customized Living Services Provided:
- Light housekeeping and meal preparation assistance for independent living.
- Meal preparation, assistance with meals
- Basic personal care assistance (e.g., bathing, toileting, dressing) for adults and children with disabilities.
- Companionship
- Medical Care (going to medical appointments etc.)
- Data collection & analysis
- Environmental assessments referrals to accessible community programs as needed.
Supported Waiver Programs
LeaCares supports several waiver programs through the S.C. Department of Health and Human Services Division of Community Long-Term Care and the S.C. Department of Disabilities and Special Needs, providing community support services and customized in-home care for adults with disabilities.”
