Customized living

Program Description:

LeaCares is an customized service provided in a person’s home funded through the Department of Health & Human Services (DHHS).  LeaCares is designed to prevent a person from needing more costly and institutional care in a nursing home or other similar type of health care facility. We serve children and adults with intellectual and physical disabilities.

 

LeaCares provides highly flexible services and supports that ensures that the individual is able to remain in their home while at the same time ensuring that all the health and personal needs are being met.  These services can be short-term or long-term depending on the needs of the individual.  All plans are highly individualized, flexible and responds to the changing needs of the person.

LeaCares supports the following waiver programs through the S.C. Department of Health and Human Services Division of Community Long-Term Care and S.C. Department of Disabilities and Special Needs.
